How to Secure Your Letter of Authorization (LOS)

Follow these steps to obtain your LOS from your HMO before booking your appointment.

Contact your HMO provider

Call your HMO's customer service hotline or visit their website to initiate the LOS request process.

Provide required information

Submit your member ID, the type of service needed, and your preferred healthcare provider details.

Wait for approval

Your HMO will review your request and send you the LOS via email or SMS, typically within 24-48 hours.

Receive your LOS

Check your email or phone for the LOS confirmation. Keep the reference number handy for your appointment.

Book your appointment

Once you have your LOS, you can proceed to book your appointment with us. Bring your LOS and valid ID on the day of your visit.
